Smart Goals Worksheet (Free Printable)

A SMART goals worksheet that gives one goal two full pages: page 1 runs the goal through the five breakdown rows (Specific, Measurable, Achievable, Relevant, Time-bound), each with one printed question and a blank answer line. Page 2 holds the goal statement and a four-row milestone table with dates and done circles. Print both, one goal per sheet, reprint for the next goal. How to use this worksheet: print both pages. Page 1: answer the five questions for one goal, in pen, no shortcuts. Page 2: copy the goal out as one statement, then fill up to four milestones with dates. Keep the pages together; page 2 is the one to look at mid-week.

Page 1: the five breakdown rows

One goal at a time. Each row asks one question; the answer goes on the line. If a question stalls you for more than a minute, that's the row to sit with. The goal isn't ready until all five rows have answers.

The five answers on page 1 feed one statement on page 2. If a row stays empty after a minute, reshape the goal before it earns a milestone.

Page 2: goal statement and milestones

The statement is the goal in one sentence, in your own words, three lines at most. The milestone table gives the goal its shape: up to four checkpoints, each with a date and a done circle to fill when it actually happens.

Each milestone should move the statement closer to done. If one doesn't clearly feed the statement, cut it and give its row to one that does.

Four opinions on using these sheets

  1. (Opinion.) One goal per sheet. Two pages sound generous until you try to fit three goals into them. Five real answers for one goal beat fifteen half-answers across three.
  2. (Opinion.) Milestones pass a yes/no test. "Book the exam" can be checked done. "Get better at math" can't. If you can't picture telling someone "done" and being believed, the milestone is too vague.
  3. (Opinion.) Date first, milestone second. Write the date before the milestone text. An undated milestone is a someday list, and the done circle means nothing without a date next to it.
  4. (Opinion.) Four rows is the cap on purpose. A goal with more than four real checkpoints usually has steps small enough to sit inside another one. Fold them in and keep the table scannable in five seconds.

What this sheet is

A planning sheet. It organizes one goal onto two pages: what the goal is, what the five breakdown questions pull out of it, and which four checkpoints mark progress. It doesn't pick goals for you and it promises nothing about outcomes. The planner it comes from works the same way: dated pages, black and white, no advice.
