Pet Emergency Contact Sheet, Free to Print

A pet emergency contact sheet the sitter reads before the first visit and the owner fills in once. One page holds an owner info box, three vet contact rows with hours, a feeding instructions box and two lines of walk notes, everything write-in. It is a pet info sheet in the plain sense: it organizes what the owner already knows so nobody reconstructs it from memory.

How to use this sheet: print one page at 100% scale (the site chrome drops out on paper) and fill it in with the owner, not for them. Every empty box is a question to ask. The sheet records their answers exactly as they say them; it decides nothing.

Pet emergency contact sheet

Pet’s name:

Facts before the first visit. Every box is a question for the owner, not a guess.

Owner info — 4 lines

Owner name:

Phone:

Address:

Backup contact + number:

House access stays practical: which gate, which door, where the key lives by name. Codes and PINs stay off paper.

Vet contacts — 3 rows

Clinic / namePhoneHours

Vet details are contact information only, copied from the owner’s card.

Feeding instructions — 4 lines

Write the schedule exactly as the owner says it, in their words. The sheet records what they said; it doesn’t improve on it.

Walk notes — 2 lines

This sheet organizes information; it gives no advice. Anything about health, food, or walks comes from the owner’s written instructions, and anything medical goes to the owner and their clinic.

Fits A4 and US Letter at 100%. Grayscale prints fine.

Four opinions on filling it in

  1. (Opinion.) Fill it in with the owner, never from memory. The client book’s setup page says to fill the profile "with the owner sitting next to you," and the same rule runs a single sheet. A recalled phone number is a guess with confidence; a read-out-loud number is a record.
  2. (Opinion.) Tape one copy on the fridge. Paper in a folder protects the paper. Paper on the fridge gets found by whoever walks in the door, which is the whole point of leaving the sheet where a substitute sitter can see it without asking anyone.
  3. (Opinion.) The hours column matters as much as the phone number. A clinic row without hours is half a row: on a Saturday the reader can’t tell whether a call goes to the clinic or straight back to the owner. Write both, and the sheet survives being read in a hurry.
  4. (Opinion.) Update it the day something changes. The product’s own note: "New number, new door, new pet: update the row the day you hear it, then fix the profile spread to match." A contact sheet with an old number is quieter than no sheet at all, because nobody double-checks the one that looks complete.

What this page is

A care-organization sheet. It holds owner and vet contact details, feeding instructions in the owner’s words, and walk notes, so the next person who walks in starts from facts instead of a text thread. It holds no guidance on health or feeding, and no claims about breeds; those live with the owner, and medical questions go to the owner and their clinic. The sheet shortens the handoff in both directions: the sitter reads it in a minute, and the owner doesn’t re-answer the same five questions for every new booking.

Where the full client book fits

This sheet covers one page of the setup. The Pet Sitter & Dog Walker Client Book covers the whole business: a two-page pet profile spread per pet, undated service calendars, a 30-row walk log, bookings, payments, supplies, incident reports. From the product README, verbatim:

"Care pages record the owner's written instructions. Nothing here is veterinary advice; medical questions go to the owner and their clinic."

"Vet contact is copied from the owner's card, nothing more."

"Personal use. Print as many copies as you need for your own business. Please don't resell or republish the files."
